The CCCP offers Advice, Information and Assistance as follows: -

• ADVICE: for who, what and where to contact.
• INFORMATION: of approximately 3000 local and regional contact numbers and 400 medical conditions written for patients.
• ASSISTANCE: to help to contact local agencies, groups, Health and Social Services.

The Community Care Communication Programme has been developed to provide information of local Professionals, Groups, Organisations and other National Help lines.  This was principally available to all local individuals, families, neighbours and carers and the people of the South Waveney area. At the request of the Waveney Primary Care Trust, it has now been extended to include the whole of the Waveney area. Information has also been included at the request of Suffolk Coastal, but at this stage is far from complete..

The database listed alphabetically under various categories includes local, regional and national contact details. These are provided for your information, advice and assistance purposes only. The inclusion on the list of a professional, practitioner, organisation, or other body does not constitute or imply any recommendation. The Community Care Communication Programme can accept no responsibility for the work of individuals or the groups listed.
